Anticoagulation Guidelines – Unfractionated Heparin Infusion

This guideline is for use by all staff who prescribe, supply, administer and monitor patients receiving anticoagulation therapy in the form of intravenous unfractionated heparin infusions on the Salford Care Organisation Site. The aim is, where possible, to standardise the doses used and recommend dose adjustments.
