The Single Point of Access processes referrals from a variety of sources including Care Homes, Hospitals, GP Practices and other services. They liaise with District Nursing teams to assist and arrange visits and clinic appointments for patients in Oldham.
The department books appointments for a number of services, these services are provided either in a clinic setting or home visits for District Nursing patients that can’t attend appointments. The services covered are as follows;
- Adult Dietetics
- Child Dietetics
- Podiatry
- Tissue Viability/Lymphoedema
- Oldham District Nursing Service (Adult only)
All patients should provide as much detail of their enquiry as soon as possible when contacting the department. This will enable the call handler to deal with their enquiry efficiently and direct it to the correct person or service.
Referrals are received from GP’s, Secondary Care, Care Homes and self-referrals.
The department has close links with all the services they receive referrals and book appointments for.
