Cancel or request to rebook your outpatient appointment at Salford
This form is to be used by patients who have an appointment scheduled at Salford Care Organisation only. Please do not use this form if your appointment is at another Care Organisation, as the request will not be processed.
If you are cancelling your appointment and have not indicated that you need a further appointment, we will discharge you back to your GP unless contact is made with the hospital within two weeks of your notice of cancellation.
If you request to rebook an appointment, we will generally be in touch in writing with a new date and time, so please specify in the comments section below any preferences you have for your new appointment, and we will do our best to accommodate them when rebooking.
