Fairfield General supporting nutrition on discharge

The Discharge Lounge at Fairfield General has been working with Community Dietitians, Porch Boxes and St Stephen’s CE Primary School Bury to help patients improve their nutrition and hydration during the first few days after leaving hospital.

Patients being discharged often need a few more days of convalescence before they feel well enough to go shopping; or their support networks may be disrupted by isolating or shielding.

Patients in need are now discharged with a food parcel, containing non-perishable items which will help see them through the first few days. The pack also contains information about how to get ongoing support and advice about meals.

The project is supported by Age UK, but open to all patients nutritionally vulnerable at the point of discharge.
