Adult Community Nursing Oldham, often referred to as the District Nursing Service, provides a range of high-quality clinical interventions for people in their own homes or community clinic settings.
The service operates 24 hours a day, 365 days a year, with a call centre to enable all patients and professionals’ access to the service, where referrals are received and triaged accordingly.
District nurses make a difference every day to the lives of the people they visit at home, in residential care homes or in clinics. They provide increasingly complex care for patients and support for family members. The team works in partnership with specialist nursing teams, advanced practitioners, GPs, social care and multi-agency teams, providing treatments and preventative measures to avoid unnecessary admissions to hospital, or to return home sooner when hospital care is necessary.
On referral to the District Nursing Service, an initial triage assessment will be carried out by our district nurse central referral team who will arrange a date and venue with you to be seen by a district nurse.
A range of nursing interventions including diabetes management, wound care, pressure ulcer prevention and management, leg ulcer treatment and management medication administration, end of life care, post-operative care, and catheter and bowel management.
Treatment Room/ Leg Ulcer Service – Clinic locations across the borough which are covered 7 days a week.
OOH District Nurses – One Team of nurses which cover the Oldham area between 5pm and 8am, visiting patients in their homes.
A district nurse will:
- assess the healthcare needs of patients and families
- monitor the quality of care they are receiving
- be professionally accountable for its delivery
Patients could be age 18 upwards, but they will often be elderly, whilst others may have been recently discharged from hospital, be terminally ill or have physical disabilities.
