We provide high quality nursing care in your usual place of residency (e.g. your own home or a residential home)
The service is for adults aged 18 and over who are housebound and registered with a Bury GP. The service is available seven days a week including bank holidays.
This includes:
- Holistic health needs assessments for patients, famililes and carers.
- Education for patients and carers to promote independence and self care.
- Holistic, planned care to support early discharge and prevent hospital admissions.
- Care for complex long term conditions, brief interventions, wound management and palliative care.
