Children's Community Nursing Service - Salford Community

The Children’s Community Nursing Team (CCN) provide specialised nursing care for children and young people aged 0-18 with acute, long term and complex health needs to prevent or minimise hospital attendance or admission.  

Referrals to our team are accepted for any child living in Salford from any professional, parent or guardian of a child or young person providing a clinical diagnosis has been made and nursing care is requested. 

We offer a wide range of services that include: 

  • Nursing care for both short and long term illnesses, including administering intravenous antibiotics in a child’s home. 

  • Support provided to families of children with chronic and complex needs with a nursing need 

  • Acute asthma care and management 

  • Support and chemotherapy administration in the home for children and young people undergoing cancer treatment 

  • Palliative and end of life care in a child’s home 

  • Support for children who require long term ventilation at home 

  • Equipment provision and training to families and educational settings  

  • Wound care including burns/scalds 

  • Transition support for young people transitioning into adult services with a nursing need 

  • Bespoke care packages for children and young people who require continuing health care 

  • Access to specialist nurses for Epilepsy, Asthma and Diabetes 

Children and young people who are referred to our team for ongoing nursing care are allocated a named nurse who will ensure that all assessments and care plans are completed and up to date and will help to co-ordinate care in the community.   

We are available 7 days per week 8am-8pm for assessment, advice, and support. 

We also have Childrens Community Nurses based in our specialist educational needs facilities who ensure all nursing needs of the students are met whilst at school/college. 

These nurses rotate into our Childrens Community Nursing  team to assist with nursing care out in the community. 

The nursing staff on the team will triage all referrals into our service. They will contact parents/guardians and undertake an assessment of need either by video call, telephone call or face to face where appropriate. 

Arrangements can be made for follow up assessments in a range of settings including home, educational settings and in our nurse led clinics.   

The nurses will liaise with other services where appropriate to ensure safe ongoing care including GP’s, allied health professionals, social care, 0-19 service, key workers and Paediatric emergency department. 

There are regular meetings between our nursing staff and GP surgeries to discuss children on our caseload and to update the GP of care and support provided.

Referrals can be made via telephone, email or via our referral form.  All demographic details including GP details and up to date contact numbers must be sent with reason for referral, brief history of illness and any other necessary information.
