Children’s Community Nursing Service - Heywood, Middleton, Rochdale (HMR) Community

The children’s community service are available every day from 8:00am to 8:00pm including bank holidays. The teams provide specialist care and treatment to children with a diagnosed acute, chronic and complex condition or illness, aiming to reduce inpatient admissions and prevent avoidable admissions for children and young people.

The teams provide nursing care and advice as appropriate, for neonates, children and young people and families.

Along with the children’s community nurses we have specialist nurses in diabetes, epilepsy and palliative care. We also have paediatric nurse practitioners who can see children and young people on the same day if they have a minor illness.

Referral Criteria 

  • Children and young people under 19 years of age 

  • Children who live in Heywood, Middleton and Rochdale (or be registered with a GP in Heywood, Middleton and Rochdale)  

  • Children and Young people who have been seen by a professional and have a diagnosis:

This list is not exhaustive and if you have a patient who you feel our nurses could support but the condition is not listed please contact the team to discuss.   

For acute illnesses the team offers an urgent response and we aim to triage all acute referrals within two hours of receiving the referral and will offer same day home visits to for referrals received up to 6pm.  

Paediatric Nurse Practitioners

The service aims to provide a clinical assessment, diagnosis and treatment for any child that has an acute illness.

Along with the children’s community nurses we have specialist nurses in diabetes, epilepsy and palliative care.    PNP Referral Criteria

Diabetes specialist Nurse

We provide care and support for children, aged from birth to 19 years, who have diabetes. We also support families and carers

We provide education and information about diabetes to allow children and young people to lead full, healthy and active lives and reduce the risk of their condition becoming worse.

We provide care in the community, such as in a clinic or health centre, or in the home. 

Epilepsy Specialist nurses

We work with children and young people, aged from birth to 19 years, who have epilepsy. We also support families and carers.

We want to make sure that children and young people receive all the support and information they need to stay healthy, live a full life, and stay out of hospital.  

We provide care in the community, such as in a health centre or clinic, or in the home.

We work closely with other professionals who help to support the child, for example paediatricians (children's doctors) or neurologists (a doctor who is an expert in conditions that affect the brain, spinal cord or nerves).

We accept referrals from educational, health, social care professionals for any child who has an epilepsy diagnosis.

Complex needs/Palliative Care Specialist Nurse

We provide palliative care to babies, children or young people with a life-limiting or life-threatening condition. We also care for babies, children and young people with complex health needs at home.

We aim to keep children and young people out of hospital, or make sure they can come home as quickly as possible.

We also support young people to transition from children to adult services. 

Each child is at the heart of the care we provide, and we will always involve the child, parents or carers. 

We will develop a tailored package of care, which includes managing symptoms and any emotional support they might need. 

This involves working closely with other professionals and services.

We want to provide high quality nursing care to make sure every child experiences the best health and quality of life possible throughout their journey.

Once our team has received a referral one of our nurses will contact the young person / carer via telephone or letter to arrange support. 

If assessment is required we will arrange an appointment to see the child in one of our clinics across the borough or at a home visit. 

We will offer follow up support as deemed necessary and young people and their families can contact the team to discuss care and seek advice as required.  

If a young person or child is coming home from hospital one of our nurses may be involved in the discharge planning meetings to ensure the family and child have all the equipment and support that you require when your child is discharged home.  

The team will refer on to other services as required and may work alongside other integrated children’s services as required or other agencies such as children’s hospice, nurseries, schools and children's social care to ensure the child, young person and their family get the care and support they require.  

Once the child or young person is ready for discharge from the CCNT the team will update the family and the registered GP for the child.     

The discharge from children’s community nursing may include transition to adult services and our nurses will aim to co-ordinate this to ensure the process is as smooth as possible.

  • Callaghan House Heywood
  • Langley Clinic Middelton
  • Whitehall Street Rochdale

Referrals can be made via letter, email or telephone by a professional 7 days a week.

Paediatric nurse practitioner service

A same day clinic appointment will be offered (subject to availability). A clinical assessment and treatment plan if appropriate will be completed by an experienced Paediatric nurse practitioner, for children who meet the below referral criteria.

To access this service you will need to ring your own GP in hours who have access to our appointment slots and out of hours ring 111 who also have access to our appointment slots if they meet the criteria  below.
