Integrated Care Centre
6th floor
New Radcliffe Street

We accept referrals from: GPs, health visitors, school nurses, community nurses, all other allied health professionals, community  and acute paediatricians.

We cannot accept self-referrals.

Once a referral is accepted, we will send you a letter inviting you to contact our service to book an appointment. We are a small service so please be aware you may have a small wait for your appointment. The appointment will be at a local Primary Care Centre and will last for around 45 minutes. The dietitian will confirm your referral and the main nutritional concerns and agree a clear care plan with you. The dietitian will decide how quickly we need to see you again, or if your child can be discharged.
