The Community Rapid Response Service is a multi-disciplinary service made up of the following professionals:
- Registered Nurses
- Community Practitioners
- Advanced Practitioners
- Trainee Advanced Practitioners
- Allied Health Professions including Physiotherapy, Occupational Therapy and Pharmacy.
- GP
The service is provided in patients own homes including residential and nursing homes within the HMR catchment area.
Assessment in your own home by an appropriate clinician within 2 days of referral, including a 2-hour response when required.
Referrals can be made into the team by any health professional.
The Team also accepts self-referrals, service users, carers and sheltered accommodation managers may refer directly via the 0300 referral line.
