Patients must be registered with an Oldham GP. Referrals can be made by GP, Health Visitors, School Nurses, Children’s Community nurses, Allied Health Professionals, other Consultants of Northern Care Alliance.
All referrals are reviewed by the triage team and inappropriate referrals are rejected with advice or re-directed to more appropriate service.
For children referred for an assessment for Autism or ADHD we require evidence of a “graduated response” – which means information about what interventions or support has been advised and tried in the first instance from the school/ preschool setting or health visiting team, or other professional involved, before a referral has been made to the service.
The graduated response information must be detailed in the referral and include information from the “Plan, Do, Review” process which details appropriate targeted strategies, advice, and interventions and that there is a need for a specialist assessment.
The Community paediatric Service works closely with other children’s teams as part of an integrated children's service including Speech and Language therapists, Children’s Community Nursing Team, Physiotherapists, Occupational Therapists, Dietetics, Health Visitors, School Nursing and Education.
