Rapid Response (Urgent care) - Bury Community

The Rapid Response Service is provided by a multi-disciplinary team of health and social care staff.

The focus is on preventing avoidable admission to acute hospital or residential care.

Referrals into the service are received from health or social care professionals and the team will respond within two hours of an appropriate referral to undertake assessment.

Care/treatment/intervention can then be provided for up to 72 hours. Necessary onward referral to community health or social care services will be made to ensure continuity of care is provided.
