We are here to support you under the Dementia Front Runner service following your discharge from hospital or return home after an A&E visit.
The Front Runner programme offers up to three weeks of free support to help you – and your carer or family – settle back safely at home.
A Lead Nurse will be your main point of contact during this time. They will work closely with other healthcare professionals to make sure your needs are met and that your care continues in a way that is right for you and your family.
Age UK Oldham can also offer additional support services, including:
- Dementia information and support
- Day care services (for up to 3 weeks if needed funded by NHS)
- Meal’s delivery service
- HandyVan home support
If you need more help beyond the three weeks, we can support you and your family in arranging longer-term services.
Our aim is to ensure you return home safely, confidently, and with the right support in place. We will work with you and your loved ones to agree on a clear plan for what happens next.
