TRIAGEHF PLUS – Medtronic UK, Manchester University FT and The Northern Care Alliance NHS Group have been Highly Commended for the HealthTech Partnership of the Year at the HSJ Partnership Awards 2021.
TriageHF Plus is a nurse-led, patient centric, clinical management tool, which leverages routinely collected health-related data from existing implanted cardiac devices, to complement clinical care, with a focus on heart failure management.
TriageHF Plus has demonstrated a significant reduction in unplanned hospital admissions and routine scheduled device follow up appointments compared to standard of care. Additionally, the end-to-end virtual monitoring means that the TriageHF Plus team have been working entirely remotely, shielding both patients and staff during the pandemic.
On being awarded this accolade, Toni Weldon, Lead Heart Failure Specialist Nurse at Bury Care Organisation, part of the Northern Care Alliance NHS Group said:
“We are absolutely delighted to have been Highly Commended in the HealthTech Partnership of the Year category. It means a great deal to all our team to be recognised in this way.
“Providing a digital solution over the past 18 months that continues to deliver outstanding patient care has been beneficial in helping to keep our vulnerable patients safe.”
