We provide assessment, diagnosis and treatment for people aged 18 and over who are housebound and have a condition that affects their independence and functioning in the community.
The service works with patients, their family and carers (formal/informal) to help patients reach maximum independence in the community. The team comprises of both Physiotherapists and Physiotherapy assistants who are all involved in the assessment of any physical issues and the implementation of an appropriate Physiotherapy Programme
The service accept respiratory conditions, orthopaedic conditions, musculoskeletal conditions, general medical conditions and any other pathologies affecting independence and function in the community.
We can also provide equipment to increase/maintain independence and safety.
The team provides interventions for adults with a range of conditions that affect their abilities to remain independent in the community.
The team works closely with other health and social teams including: the Falls team, Community Occupational Therapy team, Macmillan therapists, consultants and GPs. The service is delivered in the patient's home environment.
Patients have an initial telephone triage assessment by the team to identify their rehabilitation need. Following this, the team will either plan a face to face visit or remotely manage the patient via video consultation as appropriate.
We will work together with patients and their family/support network to agree realistic goals and expectations and provide treatment intervention to suit individual needs.
Therapy input is based upon your progress and goals will be regularly reviewed. Following completion of your intervention from community physiotherapy or if physiotherapy is not proving beneficial you will be encouraged to self manage your condition or provided with advice to access other services as appropriate. Self-management means empowering you to take control of your condition either independently or with the support of carers and will support you in managing your health conditions.
