We provide palliative care to babies, children or young people with a life-limiting or life-threatening condition. We also care for babies, children and young people with complex health needs at home.
We are part of the children's acute and ongoing needs service (CAONS).
We aim to keep children and young people out of hospital, or make sure they can come home as quickly as possible.
We also support young people to transition from children's to adult services.
Each child is at the heart of the care we provide and we will always involve the child, parents or carers.
We will develop a tailored package of care, which includes managing symptoms and any emotional support they might need.
This involves working closely with other professionals and services.
We want to provide high quality nursing care to make sure every child experiences the best health and quality of life possible throughout their journey.
