Enhanced Respiratory and Heart failure service - Rochdale Infirmary, Heywood, Middleton, Rochdale (HMR) Community

The enhanced respiratory and heart failure service is a multi-disciplinary team consisting of ACP’s, trainee ACP’s, specialist nurses, physiotherapists, therapy assistants, support workers, occupational therapist and a pharmacy technician.

We offer a variety of services, for patients with respiratory illnesses, heart failure and motor neurone disease throughout Heywood, Middleton and Rochdale.

COPD Response

We provide care to manage acutely unwell patients at home to help avoid hospital admission, where possible. We continually monitor throughout exacerbations to ensure patients improve. We provide home visits and monitoring via a virtual ward. Once a patient has initially been referred to us, a self-referral system is in place and patients can contact us for advice or a review 7 days a week 8-6. All calls are triaged by a qualified member of staff and appropriate action will be taken as per patients need.

We will help to develop an individualised care plan to help patients manage their COPD at home, this includes:-

  • Education and improving understanding of COPD
  • Breathing techniques including chest clearance, stress and anxiety management
  • Managing Medication appropriately
  • Effective Inhaler techniques
  • Support to stop smoking
  • Self management and flare up plans
  • Pulmonary Rehab

Domiciliary visits

We offer domiciliary visits to patients who are struggling with specific symptoms or multiple exacerbations to try and resolve these.

Multi-Disciplinary team meetings

We have a 2 weekly multi-disciplinary meeting with a respiratory consultant who offers further support with complex patients who are being managed in the community.

Pulmonary rehabilitation - "Keep active, breathe better"

We offer an 8-week course of gentle exercise and educational talks focused on your breathing/respiratory condition.  Our venues are in either Middleton, Castleton or Rochdale

For many people, the programme has been proven to improve your exercise tolerance, helping you breathe more efficiently, help you feel better mentally such as increasing your confidence in activities of daily living, and perhaps most importantly, gives you tools and strategies to provide you with better understanding and management of your lung condition.

Respiratory clinic

We run physiotherapy led clinics within the HMR community and offer advice on chest clearance, bronchiectasis management and Dysfunctional breathing.

Long Covid service

Patients who have ongoing symptoms 12 weeks post positive covid diagnosis are referred to the long covid service. Patients are assessed by a member of the multi-disciplinary team and discussed in a virtual MDT meeting which includes respiratory consultants, physiotherapists and psychological medicine colleagues. From here patients are offered appropriate investigations and referrals.

Heart failure

We provide care to patients in HMR community offering home visits and clinics as identified individually for the patient’s needs. We receive referrals from varying sources including GP, Heart Failure Nurses, Cardiologists, District Nurses among many more.

We see patients varying from newly diagnosed heart failure to end of life care as needed, working alongside the local INT. Patients’ diagnosis and aetiology are discussed and discussions around the benefits of pharmacological and non-pharmacological therapies. We aim to establish all patients on the recognised 4 pillars of heart failure treatment to reduce heart failure related Hospital admissions and reduce symptom burden. We link in with the local Cardiology MDT weekly undertaking discussion on an individual basis as needed.

Motor Neurone disease

We provide care for patients in Heywood, Middleton and Rochdale with Motor neurone disease offering ongoing support and physiotherapy.

An appointment will be made in advance, either by telephone call or letter. Depending on the service you may be asked to bring a list of medication.

You will have a discussion with one of the team who will work with you to achieve an appropriate plan of care and set appropriate goals and offer advice and support.
