The integrated community diabetes services (ICDS) will deliver high quality care to individuals who have diabetes and are registered with a Gp in Bury and Heywood, Middleton and Rochdale (HMR) support will also be provided to their carers.
The services multi-disciplinary team comprises of diabetes specialist nurse dietitians and consultant diabetologists and a GP with a specialist interest in diabetes
We provide the majority of your diabetes care within the local community, Depending on your needs and the complexity of your condition, you could be cared for by the Bury and HMR Diabetes Service directly, or even by your GP or practice nurse.
Only people needing the most complex and specialist services will now receive their care at Fairfield General Hospital or Rochdale Infirmary for example pregnant, Active Foot, and Diabetes Pump patients Each referral will be triage and allocated to the most appropriate clinic.
We also provide training and education for GPs and primary care clinicians to support them to effectively manage your condition and reduce the risks of any further complications.
We will also provide you with support to manage your condition through a nationally accredited programme.
