Paediatric Nurse Practitioners - Heywood, Middleton, Rochdale (HMR) Community

The service aims to provide a clinical assessment, diagnosis and treatment or a referral/signpost to the most appropriate service, for children and young people who live within the borough of Rochdale age 0-19 years if the GP is fully booked. The child/young person must have a minor illness that meets the referral criteria below. 

A same day clinic appointment will be offered (subject to availability). A clinical assessment and treatment plan if appropriate will be completed by an experienced Paediatric nurse practitioner, for children who meet the below referral criteria. To arrange an appointment contact us on 01706 676777. We also accept referrals from GM CAS/NHS 111/Bardoc, Pre ED triage and a range of other health professionals. 

Referral Criteria: 

  • Children and young people under 19 years of age 

  • Children who live in Heywood, Middleton and Rochdale (or be registered with a GP in Heywood, Middleton and Rochdale)  

  • Children who have symptoms which have been present for up to 5 days  

  • High temperatures 

  • Sore throats 

  • Coughs and colds 

  • Mild wheezing 

  • Ear aches  

  • Rashes 

  • Vomiting  

  • Loose stools 

  • Tummy aches 

  • Exacerbation of asthma  

  • Sticky eyes (In under 2’s only) 

  • Infected skin/wounds 

We are also able to give specialist respiratory advice for children with recurrent asthma/wheeze which has led to the following.  

  • 3 or more attendances at any unscheduled care provider in past 6 months 

Or 

  • 2 episodes of wheeze/asthma requiring oral steroids in 6 months 

(The GP does not need to be fully booked in this instance) 

You will be asked to wait in our child friendly waiting area’s, and greeted by one of our paediatric nurse practitioners or our healthcare support workers who will assist you in to one of our child friendly clinic rooms. We will introduce ourselves and ask for you to confirm your/your child’s/young person’s details, we will then ask you a little information and conduct a clinical assessment after gaining your/your child’s/young person’s consent. We will explain every process we need to follow. We will provide a treatment plan if appropriate and may provide an onward referral for your child/young person. We will provide you with the most up to date information we have about your condition and or treatment plan. This could be as a leaflet or as a link to your phone. 

If possible bring a list of your current medications to aid our assessment.
