General Medical Wards - Integrated Discharge Team - Transfer of Care

We are a team of Nurses who work on all wards, including Accident & Emergency, Acute Medical Unit (AMU) and Same Day Emergency Care (SDEC).

We co-ordinate safe and timely discharges for patients from all areas including End of Life care and palliative discharges to preferred place of care.

The aim of our service is to co-ordinate safe and timely discharges from hospital.

• Restarts in Package of Care
• Increase in Package of Care
• Intermediate Care
• Discharge to a care/nursing home

The Integrated Discharge Team works closely with social workers, Intermediate Care (IMC) Teams, Urgent Community Response Team (UCRT), enablement services, all wards and departments, community services and Continuing Health Care.
