We work very closely with your key worker/clinical nurse specialist and other members of the Multi-Disciplinary Team (MDT) which includes doctors, nurses, dieticians, physiotherapists and occupational therapists.
We are a key link to other professionals and services, and communicate regularly with others involved in your care, for example your GP and district nurses.
