We specialise in helping people with Chronic Obstructive Pulmonary Disease (COPD) manage their condition.
We are a team of specialist nurses, specialist physiotherapists, occupational therapists and specialist assistants.
The service responds to patients at home or their place of residence during an acute exacerbation, to prevent them from being admitted to hospital.
The service provides respiratory assessment, monitoring and treatment. We will help to develop an individualised care plan to help people manage their COPD at home.
Individualised care plans could include:
- Education and improving understanding of COPD
- Breathing techniques including chest clearance, stress and anxiety management
- Managing medication appropriately
- Effective inhaler techniques
- Support to stop smoking
- Self-management and flare up plans
- Pulmonary Rehab

If calls are received before 4pm, patients can be seen the same day if required.
