Renal - Your guide to Local Advanced Kidney Care Clinics

Introduction

Renal Services are committed to providing safe, clean and personal care to every patient, every time.

Your care will be provided from one of the 4 Northern Care Alliance Kidney Care Centres across Greater Manchester.

This leaflet is designed to explain the Kidney Care Centres in more detail and provide useful information to the lead up to the clinics commencing.

What is a Kidney Care Centre?

The Kidney Care Centre is a unit designed to deliver General Nephrology, Advanced Kidney Care Services, Transplant clinics and Dialysis. With the main hub being Salford Royal Hospital, the Kidney Care Centres are located in Oldham, Bolton and Wigan.  This will enable patients to have their care closer to home, meaning shorter journeys for appointments and more of your care in one place.

Will my consultant change?

If you are transferred to a Kidney Care Centre closer to you, this may mean you will have a new consultant taking over your care – but don’t worry, a full handover will be done, and all your clinical information will be available to your new consultant. Should there be any problems with the change, the service team at Salford Royal Hospital are on hand to assist with any concerns.

Will my nurse change?

The Chronic Kidney Disease (CKD) nursing team will remain the same and will continue to be completely involved in your care. The nursing team will have clinics at each of the Kidney Care Centres to see you alongside your consultant. You will still be able to contact them on the usual contact number should you have any queries or questions in between clinic appointments.

When will I be told my appointments are moving? How will I be told?

If your care is transferred to one of the Kidney Care Centres, you will receive a letter or phone call advising you of when your new appointment will be.

Is transport available to take me to my appointments?

If you currently book transport for your appointments, you can still do this through the usual methods. This is usually done via your local transport agency.

Will my care still be under Salford Royal Hospital

Yes. Although our Kidney Care Centres are located within the grounds or near to other Trusts, your care is still under the care of Salford Royal Hospital. This means your clinic appointments are still undertaken by a Salford Royal consultant and all your clinical information which is currently held at Salford will still be available.

Which patients will remain at Salford Royal Hospital?

Any patient that is a Salford resident, patients that need to remain at Salford for medical reasons (e.g. need to be seen more frequently) and patients who have exceptional circumstances where it is more convenient (e.g. work within Salford) will all be seen at Salford Royal Hospital.

Who can I speak to if I have any concerns?
