Diabetes Service - Oldham Community

The Oldham Diabetes Service provides high quality specialist diabetes care for adults who need advice and clinical support.

Based at Glodwick Health Centre, our multi-disciplinary community team works side-by-side with hospital-based specialists to offer you seamless diabetes care.

Visit one of our clinics across the borough and you’ll receive personally tailored care from Specialist Nurses and Dietitians, Consultant Diabetologists and specialty doctors. Home visits, including nursing and residential homes, can also be arranged where appropriate.

We’ll work with you on an individual basis to optimise management of your diabetes and reduce the risk of long-term complications. This includes effectively using oral therapies and injectable treatments.  

This support empowers all diabetes patients to look after their health and well-being while promoting self-care and independence. If you require additional support, the team works closely with other community healthcare professionals to ensure your specific needs are met. 

Once individually agreed targets have been achieved, patients are discharged back to their own GP practice. 

The service also provides education and training for healthcare professional on diabetes-related topics.

  • The service offers structured education to newly diagnosed type 2 diabetes patients.
  • Appropriate referrals to the service will be triaged to the most appropriate clinician or if multiple clinicians required a multidisciplinary clinic appointment may be offered. A choice of venues to attend will be offered. The consultation may include:
  • Individualised dietary advice
  • Agreed changes to diabetes treatment and monitoring
  • Diabetes education and advice on self-management
  • Education and support with use of flash Glucose monitoring system in patients who satisfy local and National criteria

Referrals are made to other services where required including Podiatry, district nurses, weight management / lifestyle intervention programmes and other specialist services if indicated.

If attending a clinic for the first time, you will have a full assessment including blood pressure, weight check and the clinician will take a history of your current health and medications. Any concerns you have will be discussed and you will be involved in decisions about your future treatment options and potential benefits. You will be provided a copy of the agreed management plan which will be shared with your GP.

It is helpful if you bring your current medications or recent prescription list and your blood glucose meter (if you are using one).

Royal Oldham Hospital -Diabetic Centre

You can be referred to our diabetes service via the NHS e-referral service by your GP or by the inpatient diabetes team following a hospital admission.

The service has specific referral criteria which is shared with local GP practices.
