Falls Prevention Service - Bury Community

The Falls Prevention Service provides falls prevention advice and assessment to the residents of Bury and to people registered with a Bury GP.

Individuals must be over the age of 50 and living at home, or in a nursing or residential home.

Referrals to the Falls prevention service are accepted from Clinicians, GPs and all members of the multi-disciplinary team. Self-referrals are accepted where appropriate.  

Referrals are accepted for patients age 50+ who are registered with a Bury GP. 

A history of unexplained Falls needs to be identified and individuals with a neurological diagnosis (e.g. Parkinsons, Multiple sclerosis) will not be accepted.

A referral form by Clinicians should be completed and sent to Single point of access (SPOA) Spoa.fax@nca.nhs.uk 

Self-referrals to be sent via post or e-mail buryfallsteam@nca.nhs.uk

We provide specialised assessment and treatment to people who have fallen or are at high risk of falls to help improve independence in activities of daily living.

As part of your treatment plan we may offer: rehabilitation and confidence building, provision of equipment and minor adaptations, education and advice to individuals and community groups and referral onto other services and agencies.
