Neuro Rehab Service (CNRT) incl Parkinson's - Oldham Community

The Community Neuro Rehab Team (CNRT) is a multidisciplinary team comprising of a  physiotherapist, an occupational therapist, and a rehabilitation nurse who specialise in neurological conditions.

The team provides assessment and treatment of adults with whose primary pathology is neurological, other than stroke, who are registered with an Oldham GP.

Patients referred should have an identifiable rehabilitation need and show a willingness to engage with intervention. The service is generally delivered at the patient's home but some patients are also seen in clinic. The team work closely with other health, social and vocational services.

The Parkinson's Disease Nurse Specialist (PDNS) works as part of the team providing continuous care for people with parkinson's disease and related conditions.

The parkinson's service has an open referral system and receives referrals from patients, carers or health care professionals. The parkinson nurse provides on-going assessment, advice on symptom control, medicine management, education, liaison and onward referral for patients and their family/carers.

The nurse works closely with the specialist consultant physicians and neurologists at the Royal Oldham Hospital and care is delivered to meet patients' needs and preferences in clinic or in their own home.
