Preparing to leave hospital
We understand that most people want to return home from hospital as soon as they’re well enough. That’s why, from the moment you’re admitted, our team begins planning for your safe discharge. Your nursing team will talk with you about what needs to be arranged, so you can prepare in advance.
Leaving the ward
Once you're medically fit to leave, you'll be discharged. On discharge day, you may be moved to a discharge lounge. This setting is usually staffed by nurses and designed to support patients waiting for transport or family pick-up.
What to expect when discharged home
If you're going home or to a care facility, several steps may be involved. For routine discharges, please:
- Organise your own transport unless hospital transport is deemed necessary.
- Ask any questions about your care or follow-up before leaving.
- Collect all personal items and valuables.
- Take home any belongings used during your stay.
- Ensure you receive any prescribed medications.
Medication
You’ll be given a one-week supply of any prescribed medication. If your treatment needs to continue, please contact your GP for a repeat prescription. Any charges for medication will be discussed with you beforehand.
Support following discharge
If you need additional support, such as rehabilitation or home care this will be arranged by your ward team or a discharge coordinator. Support may involve:
- Social services
- District nurses
- Occupational therapists
- Physiotherapists
- Dietitians
- Dementia care specialists
You’ll be informed of any care plans before leaving the ward.
Fit notes and claim forms
If you need a fit note or help with claim forms, speak to a member of the ward team. Some forms may incur a charge, and staff will advise you accordingly.
Transport arrangements
Patients are generally expected to arrange their own transport home. In exceptional cases, hospital transport may be provided. If you need help, please speak to your nursing team.
Follow-up appointments
If you require a follow-up, this will either be scheduled before you leave or sent to you by post shortly after discharge.
