About Health and Social Care Assessments 

Community learning disability team - The community learning disability team work in Salford. They help people who have learning disabilities; live in Salford; live in other places and have their care paid for by Salford social care. Care - The Care Act 2014 is a law. A law is a set of rules we must follow to help keep people safe. Social Care workers must follow the rules in the care Act 2014. Assessment - one of the rules is about having a health and social assessment.

Support plan - an assessment is when we find out what things you can do well; what things you need support with. We use this information to make a plan to help you. Reading - read this leaflet with someone who supports you. If you have any questions you can ask them for help

How do I get a Health and Social Care Assessment? 

Help - The Care Act says people can ask for a health and social care assessment. You are someone who supports you, can do this on the telephone. Contact - you need to call Adult Social Care. The number is 0161 206 0604. Call takers - the person on the phone will ask you some questions. The questions help us work out if you need an assessment. Appointment - a social care worker will make an appointment to see you.

Waiting list - sometimes people have to wait for an appointment. Some people wait a short time. Some people wait for a longer time. Help - if anything changes while you are waiting that is OK. Please ring us and let us know. You can call us on 0161 206 0604.

What happens during the Health and Social Care Assessment?

Assessment - on the day of the assessment. The social care worker will visit you. My support - someone who supports you can come to this meeting. This could be a family member, a friend or paid support. They can help you during the meeting. Ask questions - THe social care worker will ask you some questions. Things like what is working well? What do you need help with?

The Social Care Worker will ask you about: 

Health check - your health. Healthy eating - your eating and drinking. Keep clean - Your personal care. This is things like keeping clean, getting dressed and going to the toilet. Safe home - Keeping yourself safe at home.

Cleaning - looking after your home. This could be keeping things clean. Support from family/friends - your friends and family. Work - your education, work and volunteering. Going out - going out in the community. They will ask you about using public transport.

Children - If you look after children. Talk to the social care worker will talk about the things you need help with. Help - the social care worker will talk about how they can help you.

How can the Social Care Worker help you?

Care at home - we can help people to have support at home. Going out - we can help people to go out. Told about community services - We can help people go to places near where tehy live. Places like a day service, groups or clubs. Sometimes, we call these 'community services'. Supported living - we can help people move home. We can help them get the support they need.

Respite - we can help people go to respite services. These are places you can go for a short break. Work - we can help people find out about employment support services. Employment support services can help people to work. Adult education - we can help people find out about education services. Education services help people who want to learn more things, like going to college or a course. My support - the social care worker will talk to you about your help and support

What happens after the Health and Social Care Assessment? 

Help writing your plan - the social care worker will work with you to make a support plan. A support plan has ideas to help you. Agree - your social care worker will agree how much money you an get. The money is used to pay for your support. We call this a personal budget. Ask questions - You will use your personal budget to pay for your support. You can ask ask social care services to help you do this. Direct payment - You can ask social care services to give you money from your personal budget. You will use money to pay your support. We call this a direct payment. Your social care worker can tell you more about direct payments.

Money - some support services cost money. Some people pay a small amount of money. Some people pay a bit more money. Help - the Fairer Charging Team are apart of Salford Social Care Service. Your social worker will ask the team to help you. Money - the Fairer Charging Team will find out if you can afford to pay for your support; work out how much money you need to pay for your support. We call this finding out an assessment. My support - if you find the assessment of support planning difficult that is OK. You can ask a friend, family member or advocate to help you.
