We provide up to six weeks rehabilitation and therapy at home, to help you improve your independence.
If everyday living tasks are very difficult or you have had falls at home, you may have been referred you to our service.
You may also have been referred to our service if you have been in hospital or an intermediate care rehabilitation unit and need some support when you return home.
What we do
We look at your needs and your goals (what you want to be able to do) in day-to-day life. This is called an assessment.
We build a personalised rehabilitation and therapy programme with you. This identifies what you want to work towards and how we are going to help you achieve them.
The programme may include:
- Trying new ways of doing things or using equipment to help you to do daily tasks
- Exercises to improve your strength, balance, flexibility and stamina
- Asking your carers, relatives and friends to get involved
We will regularly review how you are doing and, if needed, we will adjust your programme. This is called a review. The programme can last for up to six weeks. Many people stop before then because they achieve their goals in a shorter timeframe.
Who helps you?
Advanced Practitioners - Look at whether your health or medications are limiting the progress of your rehabilitation.
Assistant Practitioners/Rehabilitation Technicians - Help you to carry out exercise programmes and practice activities within your daily life.
Occupational Therapists - Help you find different ways of doing everyday living tasks, sometimes using items of equipment.
Physiotherapists - Provide exercise programmes targeting your specific areas of needs.
Social Workers - Coordinate and help you with wider aspects of your life. If you need longer-term support, the social worker will talk to you about this.
Appointments
Once we have received your details, we will usually contact you within two weeks to arrange a first appointment. If we are unable to contact you within two weeks, or you are admitted to hospital for longer than two weeks, we will stop the service and let your GP know.

If you are unable to keep your appointment or you are unwell on the day of your appointment it is very important to contact us on 0161 906 1550 or email ics.rehab@nca.nhs.uk
We have a duty of care to make sure you are safe.
If you do not contact us to cancel your appointment and we cannot get in contact with you, or find out where you are, we may contact the police.
To make sure you are safe, they will take appropriate action, which could include breaking down your front door. The repair would be your responsibility.
What we expect from you
We have a duty to provide a safe and healthy environment for all staff.
You can help us by not smoking, drinking alcohol or taking illegal drugs just before or during our visits; putting pets in another room and being prepared for us.
Our staff will not accept aggression, violence or threatening behaviour, so please respect them.

If you happy with our service or have concerns about our care speak to a member of staff or the manager first.
If this doesn’t help, the Patient Advisory Liaison Service (PALS) is available to assist with problems and will liaise with staff on your behalf. They can be contacted on 0161 206 2003.
