Integrated & Community Services - Oldham Community Nutrition and Dietetics Team

The Oldham Community Nutrition & Dietetic service in Oldham is run by a team of NHS staff, including:

• Dietitians
• Dietetic Assistants
• Admin

Dietitians are qualified and regulated health professionals, who assess, diagnose and treat dietary and nutritional problems at an individual level for adult patients (18 years +) with an Oldham GP.

We use the most up-to-date guidance and translate this into practical information to enable people to make appropriate lifestyle and food choices.

Dietitians and dietetic assistants work alongside health care staff (including community nurses, speech and language therapists, occupational therapists) to support your recovery

How to refer to us

There is a criteria for referrals to our service. You can be referred by a healthcare professional, nurse or GP if they feel you require dietetic support and meet the criteria.

All patients are seen on a priority basis, dependent on clinical need.

Patients eligible for a home visit by our service are those who meet one or more of the following:

• Patients who are bedbound/nursed in bed at all times
• Patients who require hoisting in order to move or travel

What do we do?

After you have been referred, you will either be contacted or be invited to make contact with our dietetic team to arrange an appointment. We provide clinic appointments and home visits.

What to expect from the Dietetics Team

At your appointment, we will:

• Assess your dietary needs
• Develop specific, measured, achievable, realistic and timely goals/care plan to suit you
• Refer you to other professionals if needed
• Signpost you to other services if necessary

Interpreters

If you would like an interpreter, staff will arrange one.
