What we do
We provide support during an assessment period which focuses on rehabilitation, therapy and reablement at home, to help you improve your independence.
You have been referred to our service for rehab and reablement because you have recently been discharged from Hospital, or an Intermediate Care Unit (within the last 2 weeks) and/or are finding everyday tasks more difficult than usual.
We look at your needs and your goals (what you want to be able to do) in day-to-day life (this is called an Assessment). We then build a personalised rehabilitation and therapy programme with you. This identifies what goals you want to work towards and how we are going to help you achieve them.
The programme may include:
- Trying new ways of doing things or using equipment to help you to do daily tasks
- Exercises to improve your strength, balance, flexibility and stamina
- Asking your carers, relatives, and friends to get involved
- The Reablement Support Worker may observe, encourage, prompt new or re-learn skills
- Help with personal care e.g. washing, dressing and going to the toilet
- Help with domestic tasks e.g. food and drink preparation and light tasks
We will regularly review how you are doing and, if needed, we will adjust your programme (this is called a Review). The daily support from Reablement Support Workers can last for up to six weeks, however, many people stop before then because they achieve their goals in a shorter timeframe.
Who will help you?
Assistant Practitioners/Rehabilitation Technicians - Help you to carry out exercise programmes and practice activities within your daily life.
Occupational Therapists - Help you find different ways of doing everyday living tasks, sometimes using items of equipment.
Physiotherapists - Provide exercise programmes targeting your specific areas of needs.
Reablement Co-ordinators - Coordinate/arrange the delivery of your care plan by reablement support workers and review your needs regularly. If you need longer-term support, they will discuss this with you, your family or carer at the end of the period of reablement and refer you to a Social Worker.
Reablement Support Workers - Provide support up to 4 times daily with meal preparation, personal care, and other activities of daily living. Their role is to help improve your independence.
Pharmacists and Pharmacy Technicians - Will ensure your medications needs are met and are not limiting the progress of your rehabilitation and reablement and check that you are managing to take them as prescribed.
Appointments
Once we have received your details, if we have not already seen you at home as part of hospital discharge, we will contact you to arrange a first appointment. If we are unable to contact you, or you are admitted to hospital for longer than a week, we will stop the service.

If you are unable to keep your appointment or are unwell on the day of your appointment it is very important to contact us on the above phone number or email address to let us know.
If you feel unwell, please contact your GP or alternatively NHS111. In an emergency, please dial 999.
We have a duty of care to make sure you are safe. If we cannot get in contact with you for a planned visit, call or appointment and you did not cancel the appointment, we will contact your family initially, where appropriate, to check that you are OK. If we cannot locate where you are and are concerned, we may contact the police.
To make sure you are safe, they will take appropriate action, which if assessed to be an emergency this could include breaking down your front door. The repair would be your responsibility.
What we expect from you
We have a duty to provide a safe and healthy environment for all staff. You can help us by not smoking, drinking alcohol, or taking illegal drugs just before or during our visits, putting pets in another room and being prepared for us. Our staff will not accept aggression, violence, or threatening behaviour, so please respect them.
Other information and useful numbers
All our staff strive to make sure that every patient's experience is safe, clean, and provides personalised care, every time.
If at any time you think your care is not safe, clean or personal, you should speak to a member of staff or the manager first. If this doesn't help, the Patient Advisory Liaison Service (PALS) is available to assist with problems and will liaise with staff on your behalf. They can be contacted on 0161 778 5665. This line also has an answer service out of hours or when the department is busy.
