About the service

The liaison team are here to help patients, staff and families, by ensuring and reinforcing that the hospital is offering a person centered, reasonable adjustment and safe patient journey.

The service has a focus on supporting a positive patient experience and positive health outcomes for individuals with a learning disability.

We support all 4 hospitals managed by Northern Care Alliance. This includes Salford Royal Hospital, Oldham Royal Hospital, Fairfield General Hospital and Rochdale Infirmary.

Who is the service for?

Anyone with a learning disability, their family, carers and Northern Care Alliance staff who need support and advice. The service supports patients throughout their life.

How can we help?

We want to make sure people with learning disabilities have a good experience of using hospital services. We can do this by:

  • Supporting people with a learning disability through their outpatient appointment or stay in hospital
  • Support patients' understanding of care and treatment pathways
  • Make sure reasonable adjustments are implemented
  • Listen to and involve family and carers in the patients care
  • Support the development of easy read information where required
  • Promote the use of hospital passports

How to contact/refer into the team?

Anyone can refer by telephone or email. This should be the first contact. Members of the clinical team can complete referrals directly to the service via the electronic patient records.
